Description of Services:

Supports for individuals and their families, who have been diagnosed with cancer and other life-limiting illnesses * referral to other community services

provides supportive care based on clients and family needs through supportive care coordination and trained volunteers
practical psychological, emotional, social, spiritual and bereavement

Services are provided at home, hospital, hospice and long term care homes

Education and consultation to service providers offering palliative and end-of-life care in home, hospital, hospice suites or long term care homes
