Description of Services:
Community based GAIN clinic provides comprehensive geriatric assessments to address the complex needs of many older adults. Assessments are done in-clinic or in-home, pending eligibility. The inter-professional teams work with older people and their families to develop care plans that support frail seniors with complex health issues so they can remain living at home. Staff include:
- Nurse Practitioner(s)
- Care Coordinators
- Occupational Therapist
- Social Worker
- Pharmacist
- Behavioural Support (BSO) Nurse
- geriatrician support
- administrative team members
