Description of Services:
Provides specialized transitional planning for youth who are funded by the Ministry of Children, Community and Social Services in the regions of Durham, Kawartha Lakes, Peterborough, Northumberland and Haliburton, to prepare the individual, their family, and their team for the transition from child to adult-funded support and services * TAY Coordinators work to ensure services and supports meet legislative requirements and work in collaboration with the individual’s circle of care to create service opportunities * TAY Coordinators also work to profile youth issues and gaps in service through committee membership, focus groups, and training events * Program provides intensive transitional planning coordination for youth 14-21 years old, striving to attain service continuity and best practice as well as case management, residential support and community participation support to those adults awaiting transfer to adult supports and services.
