Description of Services:
Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, nurse educators, diabetes educator, chiropodist (foot care specialist), social workers, dietitians, kinesiologist (health professional focused on the movement of the body), and respiratory therapist Services and programs for registered patients - Primary care, foot care, one-on-one counselling services, chronic disease management, smoking cessation and memory clinic Services available to both patients and the general public - Health promotion programs on a variety of health topics including heart disease, diabetes, hypertension, cancer, arthritis, and falls prevention * screening programs * physical activity and healthy eating programs * breastfeeding support * virtual prenatal classes.
