Description of Services:
Program coordinator will provide a one-time individualized consultation for care partners and people with Parkinson's disease (PD) * assist them to optimize benefits of Parkinson's medications, to recognize their motor and non-motor symptoms related to Parkinson's
For those who are also able to participate in group education, will be invited to join our 8 weekly education and exercise program * objective is to learn strategies and healthy lifestyle to live well with Parkinson's disease and to delay disease progression * presentations provided by our inter-professional team including physiotherapist, pharmacist, occupational therapist, recreational therapist, registered dietician and social worker
