Description of Services:
Supports for people living with prediabetes or type 2 diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff * chiropody services available with referral from your physician or diabetes educator
- weekly virtual or in person groups - for people new to diabetes or a refresher
- insulin starts
- glucose screening clinics
- nutritional counseling (healthy eating, meal planning recommendations)
- post-gestational
- foot care and chiropody services
- weight management
