Description of Services:
Multi-service agency, with three supportive housing sites and a community health
centre * deliver health and social care programs that include primary
healthcare services, health promotion and prevention programs, attendant care
and personal support and community development initiatives, particularly for vulnerable
individuals with complex needs * focus on youth, complex seniors, adults with
physical disabilities, hidden homeless, socially isolated, racialized
communities * health professionals assess and treat non-life-threatening
injuries or illnesses * counselling * referrals to other local health services
and personal development groups

- Health Promotion and Prevention Programs Offer a wide range of health promotion and prevention programming that includes focus on exercise, nutrition, social inclusion, mental health, and arts-based initiatives * also deliver a range of programming to address clients' social determinants of health needs, including housing, food security, and employment
- Mid-Toronto Diabetes Education Program Supports for people living with prediabetes or type 2 diabetes * provides free consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices by a team of certified diabetes educators that includes a registered nurse and registered dietitian
- Primary Care Basic health care from the doctor, nurse or nurse practitioner * nutritional services, physiotherapy or education on things like diabetes * also involve counselling and care coordination
