Individuals 18 years of age and older
- Must have a valid Ontario Health Insurance Plan (OHIP) card
- Must have health care needs that cannot be met with any combination of caregiving, care in your home or care in your community and
- Must have health care needs that can be met in a long term care home

Program - Some programs may require additional fees; Service - Accommodation; Fee assistance available for Basic accommodations; assistance is provided by Ontario Health
