Description of Services:

Provides support to youth with developmental disabilities to create a transitional plan for life after graduating from high school

Assists families to identify and access resources through transitional planning is school partnership and into community life

Assists with applications and interviews such as Developmental Services Ontario (DSO) housing, passport, Ontario Disability Support Pension and access to adult services, Clinical/Surrey Place

Offers resource planning for accessing further education, employment, training opportunities and community resources
