Supports for people living with pre-diabetes or diabetes
- Assists individuals to cope with diabetes and prevent complications
- Offers education and self-management support to fit an individual's lifestyle
- Facilitates client support groups
- Provides access to a Registered Nurse and Registered Dietitian
- Coordinates foot care workshops
