- Type 2 Diabetes (diet-controlled, pills, insulin or any combination of treatment)
- Gestational Diabetes
- Type 1 / Type 2 Pregnancy
- Impaired Glucose Tolerance
- Impaired Fasting Glucose
- High Risk
Child and Youth 17 years of age and younger
- Type 1 Diabetes
- Type 2 Diabetes (on insulin)
- Type 2 Diabetes (no insulin)

Description of Services:
Supports for individuals with prediabetes or diabetes
Programs and services include:
- Offers a diabetes management clinic to assist individuals in understanding and managing their illness
- Program services are provided through individual sessions, group sessions and health promotion activities
Programs and services include:
- Client support groups
- Community screening
- Exercise programs
- Foot care workshops/clinics
- Health fairs
- Presentations to the public and to health care providers
- School and daycare programs and visits
- Provides consultations and services for a half day per week on a walk-in basis
