The following documentation is required to complete application:
Long Lake 58 Social Services will notify applicant of decision for eligibility in writing; if applicant disagrees with decision, a written request to have the decision reviewed must be received within thirty days and an Internal Review will be conducted within ten business days, after which time the applicant may appeal to the Social Benefits Tribunal
The following documentation is required to complete application:
- Proof of income for the applicant, common-law partner and children over age of 18 years of age
- Recent income tax return or notice of assessment
- Copy of all household expenses such as home rental lease, heat and hydro bills etc
- Health card
- Proof of school attendance
- Proof of assets, such as financial institutions, banks, credit unions (ie Caisse Populaire), vehicle ownership, life insurance
- Information about other income and money, such as a pension
- Proof of identity, such as birth certificate for all family members
- Proof of Social Insurance Number
- Record of Employment and/or most recent pay stubs

Individuals must:
- be a resident of Ontario
- be in immediate financial need to help pay for food, clothing and other personal items and housing costs associated with rent, repairs and heat arrears
- be willing to take part in activities that will help find employment and help with the well being of self and family
- sign a Participation Agreement
