Care Navigator conducts eligibility assessments and collaborates with seniors and their caregivers to plan and coordinate services align with their needs, goals, and desired outcomes * services may be delivered at the seniors's residence, another designated location, or via telephone * Client Intervention and Assistance Service offers assessments, case management, service navigation, advocacy, information and referral, and aid to older adults and their caregivers who are vulnerable or at risk * Care Navigators help clients connect with suitable community resources
