Apartments with 24 hour personal care
  • on-site personal support workers
  • housekeeping
  • meal preparation
  • medication reminders and assistance
  • emergency response
  • security checks
  • escorts to appointments
  • social and recreational activities

Case Management Support
- Provides system navigation * crisis intervention * psychosocial support * health education and promotion

Reintegration Program:
3 transitional units * short term supportive housing services * case management support for older adults (59+) with serious mental health, substance use or addictions challenges who are presently in hospital (Ed/ALC patients) and require transitional support services prior to achieving permanent housing in the community
