Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, nurses, child and youth social worker, registered dietitians, addiction counselor, case worker, chiropodist and pharmacists

• mental health promotion - one-on-one and group therapy

• CBT (Cognitive Behavior Therapy) - depression and anxiety, and insomnia

• NP Led homebound program
