Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, nurses, mental health workers, case manager, chiropodist (foot care specialist), pharmacist and dietitian

• mental health promotion - group support, psychotherapy, resource counselling, advocacy, and system navigation
• Stop For FHT (family health teams) - smoking cessation program
