Application
Referral from a physician or nurse practitioner is required for many programs and should be initiated through our Central Navigation.
Eligibility - Population(s) Served
Previous fall in the last 12 months OR fear of falling* Ambulatory with or without gait aid *Able to stand unassisted* Cognitive ability/interest in participating in group discussion * Must be aged 65 or older to participate

Description of Services:
Falls prevention classes for older adults and people with mobility issues or chronic conditions
The program is divided into two streams
Falls Prevention Program
- A 10-week program offering 1 hour sessions 1 per week of combined education and exercise
- 20 minute discussion of fall prevention education and strategies (i.e. Identification of falls risk factors, movement strategies, reaching, bending, turning, environmental factors, community mobility)
- 40 minutes of fall prevention exercise and balance activities (focusing on core stability, leg strength and challenging balance)- Group size: Maximum 10 participants per group.
Maintenance Exercise program
The maintenance component of the Falls Prevention Program is aimed at supporting clients in maintaining the abilities they have gained in the Falls Prevention program * Classes aimed to support individuals that are at risk for deconditioning and could benefit from staying fit due to an ongoing chronic condition
