Must be referred by a northern Ontario health provider (physician, dentist, optometrist, chiropractor, midwife or nurse practitioner)

Application form must be filled out by you, the referring Northern health care provider and the specialist or designated health care facility)
- Must have a valid Ontario health care
- Health care provider must be a ministry designated facility
