Caregiver, professional or client must make referral through the Toronto ABI Network

Eligibility - Population(s) Served

Adults 18 years or older who have moderate to severe acquired brain injury * must be medically stable * must be currently living in, or preparing to return to, the community * must have a family doctor

Hospital-based outpatient transitional day program with a focus on acquired brain injury rehabilitation * recreational activities, social support, and skills training.
