In-home supports for the activities of daily living for high risk older adults (frail or cognitively impaired) * services may not exceed 180 hours per month
- personal support services
- homemaking
- professional services (occupational therapy, physiotherapy, dietitian, social worker)

- emergency urgent response
- care coordination
