Description of Services:
An in-home support and self-management coaching and monitoring program for individuals diagnosed with chronic obstructive pulmonary disease (COPD) and/ or chronic heart failure (CHF) * helps participants gain the skills and confidence to better manage their COPD or CHF at home.
Easy to use equipment and health coaching helps COPD and CHF patients enjoy the best possible health while staying in their own home * the program helps them become active partners in their own health care, teaching them the signs of early disease flare-ups, and reducing trips to the hospital and emergency department
Who is eligible for the program?
- no cost, six month program teaches disease skills / supports to COPD and CHF patients within home setting
- daily (M-F), remote monitoring of O2 sat, BP, weight, and symptoms teaches patient early exacerbation recognition and management * can include diabetes monitoring as co-morbidity
- primary care provider collaboration results in better medication / treatment compliance / status trending
- regular telephone disease/lifestyle self-management coaching sessions are based on relevance to patient’s disease and program goal(s)

Who is eligible for the program?
- diagnosis of COPD / CHF
- cognition fully intact to mild impairment or caregiver support in home
- patient and/or caregiver willing and able to participate in health coaching and data sending
- 18 years of age or older
