Description of Services:
Provides outpatient rehabilitation services * programming includes interventional procedures, time-limited and goal-oriented rehabilitation services (up to 6 months)
Ontario Telemedicine Network is utilized to provide some rehabilitation interventions, consultation and follow-up services to minimize the need for travel
Pain duration must be lasting longer than three months and patient has not responded to other pain management therapies * chronic pain conditions include:chronic neck
shoulder or back pain
whiplash injuries
fibromyalgia
repetitive strain injury
chronic regional pain syndrome
post stroke or central pain
neuropathic pain
The interdisciplinary team consists of the Clinical Manager, Program Coordinator, Clinical Lead, Administrative Support, Case Manager, Occupational Therapist, Pain Specialists - Physicians, Pharmacist, Physiotherapists, Recreation Therapist, a Psychological Associate, Registered Nurses, and a Registered Social Workers
