Description of Services:
The GAIN Community Team supports frail older adults, (generally 75 years plus) adults living at home or in retirement, or, residents who are experiencing changes in support needs, safety concerns. GAIN teams provide specialized geriatric care to support frail seniors living at home or in retirement residences, who have multiple complex medical and social problems including:
The Carea CHC GAIN team consists of nurse practitioners, occupational therapist, social worker, pharmacist, personal support workers, dietitions, dedicated GAIN/Ontario Health atHome Care Coordinator and, Behavioural Supports Ontario (BSO) clinician, working in collaboration with consulting Geriatricians and other specialists.

- Responsive behaviours
- Decreased function
- Falls or risk of falls
- Impaired mobility
- Incontinence and/or
- Multiple medications
