Eligibility Criteria:
Individuals diagnosed with a serious mental illness such as schizophrenia, bipolar effective disorder, psychotic disorder and schizoaffective disorder and who have had multiple psychiatric hospitalizations.

Description of Services:
Provides comprehensive psychiatric treatment and psychosocial rehabilitation. Provides community based treatment and support with 24-hour availability. Clients must be heavy users of crisis, emergency departments, and/or inpatient hospital services.
