Description of Services:
Self-Management and Exercise Program for stroke survivors and their care partners (family and friends)
- Focuses on education, goal-setting, and exercise
- Helps individuals learn about strategies to manage the daily challenges of living with a stroke
- Holds sessions three times a year, in Jan, Apr, and Sep
- Provides up to 6 participants in Thunder Bay (host site) and 2-6 participants at 1-2 regional sites via Ontario Telemedicine Network
