Description of Services:
Diabetes Nurse Educators and Registered Dietitians offer self-management education & support for people living with prediabetes and Type 2 diabetes. Available to those newly diagnosed or living with these conditions for years, including people starting or already using insulin.
1:1 appointments (by healthcare provider referral) and group sessions available.
