Description of Services:
Community-based service for youth and young adults with a developmental disability, which will assist them in the transition process from school to life as adults in the community
Services include:
Services include:
- access to a transition coordinator
- supports and resources based on needs and interests
- assist in completing application process for Developmental Services Ontario
- assist in completing referral package and application for Ontario Disability Support Program
- in conjunction with the school system, completion of Integrated Transition Plan and updated on a yearly basis
- skill building
