Assessment, treatment and referrals for people seeking mental health and addictions support. Also including problem gambling issues

- walk in crisis supports
- crisis intervention
- virtual supports
- mobile crisis teams
- police partnerships
- transitional case management
- counselling
- community wellness programs
- housing supports
- transitional housing and addiction medicine supports
- Addiction Transitional Case Management Service - Assessment and Referral Program facilitates discharge process from St Thomas Elgin General Hospital * ensures patients have linkages to appropriate addiction and mental health services and supports upon discharge * patient works with Addiction Transitional Case Manager to create and implement plan for care
- Back On Track Program Provincially mandated program for convicted impaired drivers and drivers who receive two or more provincial alcohol or drug related driving offences
- Community Opioid Addiction Program Offers direct client services and community education for people with opioid addictions * assessment and screening required for entry into the treatment program
- Community Treatment Order Case Management Intensive case management services to people with a serious mental illness who are subject to a Community Treatment Order (CTO) issued by a psychiatrist
- Crisis Intervention Walk-in and mobile assistance to people who are experiencing a mental health crisis
- Hospital Walk-In Crisis Support Walk-in days, evening and weekend service at St Thomas Elgin General Hospital to people who are experiencing a mental health crisis
- Housing Support Drop-In Assistance with applying to subsidized housing in St Thomas and Elgin County * access to resources, including apartment application requirements, current apartment and room rental listings, information on subjects related to obtaining and maintaining housing * linking with appropriate services such as income support
- Intensive Case Management Intensive case management services to people with a serious mental illness
- Substance Use Services Assesses people with substance use concerns using standardized assessment tools * develop treatment plans, including referrals to other organizations where appropriate
