Open to referrals from health care provider or self-referral
New patients must complete intake process before becoming a registered client of the clinic

Individuals aged 16 years and older with an opiate addiction

Individuals without coverage must pay a monthly payment

Provides medication and therapeutic treatment for opiate addiction

Based on an intensive outpatient model

Offers initial intake screening, appropriate lab tests, regular medical follow-ups and regular suboxone dosing
