Application

Target communities include the Cambodian, Chinese (Cantonese, Mandarin), Korean and Vietnamese communities. Acceptance for service is subject to the outcome of the intake assessment. Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.

Description of Services:

Case Managers help individuals and their families to access affordable and stable housing, live in the community and achieve their personal recovery goals. Case management services are provided through individualized supports such as problem solving, risk management, coping with emotions, managing mental health issues, linking to community resources, developing positive relationships and life skills training.
