Description of Services:
North Algoma Rapid Access Addiction Medicine (RAAM) is a rapid access clinic located within the Wawa Family Health Team. It is a clinic for individuals with substance use issues that require specialized addiction medicine support.
An addiction-trained nurse practitioner will provide, assessment, case management, referral for counselling support, as well as prescribe appropriate addiction medications or connect patients to community treatment programs.
Once individuals are stabilized, the goal is for them to be referred back to primary care for long-term follow-up.
The Nurse Practitioner can provide ongoing support by being available to family physicians for re-assessments, consultations, and advice about addiction care.
In this model, addiction specialists, hospital staff, family doctors, and community service agents work together to provide seamless and accessible care to patients with substance use disorder.
