Eligibility - Population(s) Served

  • Individuals 18 years of age and older with chronic or long-term health conditions and their caregivers

  • Participants must be able to set goals and function in a group setting; for example, the program would not be appropriate for someone with significant dementia or memory impairment, or for someone who would be disruptive in a group

Description of Services:

  • Offers a 6-week workshop for individuals living with chronic or long-term health conditions such as diabetes, arthritis, stroke, cancer, colitis, lung disease, fibromyalgia, Parkinson's, heart disease and other conditions
  • Educates individuals on how to manage their symptoms to become more confident and improve their quality of life
  • Offers workshops online
  • Workshop topics include:
    • Coping with difficult emotions
    • Pain and fatigue management
    • Developing action plans and problem solving skills
    • Exercise and nutrition
    • Managing Medications
    • Making treatment decisions
    • Working with the health care team
