Eligibility - Population(s) Served
- Individuals 18 years of age and older with chronic or long-term health conditions and their caregivers
- Participants must be able to set goals and function in a group setting; for example, the program would not be appropriate for someone with significant dementia or memory impairment, or for someone who would be disruptive in a group
