Medical
care team focused on family and community health * provides acute and chronic
disease management, health promotion and disease prevention * team includes
family doctors, nurse practitioner, registered nurse, registered dietitian,
registered kinesiologist, palliative care nurse navigator, social worker,
occupational therapist, addictions/mental health counselor, pharmacist, and
psychologist
