Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes a nurse practitioner, registered nurse, social worker, registered dieticians, registered kinesiologist, registered respiratory therapist and occupational therapists

Hearts in Motion Cardiac Rehabilitation Program Medically supervised rehabilitation program for people who have had a cardiac event within the past two years * in person and virtual options available
