Provides end-of-life care and grief services

Residential Care:

24 hour nursing and personal support
homelike environment with 10 private rooms
comprehensive pain and symptom management
supportive care

Grief Services:

emotional, social and spiritual support to individuals facing death and dying and their families
one-on-one grief sessions
grief support groups

Supportive Care:

connecting residents with faith leaders
support for spiritual/religious rituals
support for issues regarding death and loss

Shared Care Services:

Connect program links to Palliative Care Algoma services

Advance Care Planning (ACP) services

ACP Champion for Algoma housed within this program
