Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners (NP), registered practical nurses (RPN), social worker, and administrative staff * seasonal flu vaccine clinic by appointment * monthly respiratory therapy, memory and hearing assessment clinics * foot care services (non-OHIP) * community programs/workshops
