Medical clinic focused on family and community health * nurse practitioner works with other health professionals to assess and treat non-life threatening injuries or illnesses

Chronic Disease Prevention and Management Program - Sessions to learn ongoing self-care * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions
