Eligibility - Population(s) Served

  • Individuals 18 years of age and older with specific needs such as disability, illness or injury and require assistance to live in own residence

  • Seniors 65 years of age and older

Service - Accommodation - rent geared to income; Minimal monthly fee for activities and outings

Description of Services:

Provides supportive housing and personal support to enable individuals to live independently in their own apartment
  • Promotes mental and physical health
  • Provides activities and outings for program members
Offers assistance from an on-site support worker with maintaining and developing daily living skills including:
  • Grocery shopping
  • Health care coordination
  • Healthy meal planning and preparation
  • Housekeeping
  • Medication reminders
  • Personal care
  • Security Checks
