Description of Services:

Supports for individuals living with prediabetes or diabetes
  • Provides education and services for the prevention, management and treatment of diabetes
  • Incorporates both traditional and contemporary healthy living services focused on reducing the risk of developing Type 2 diabetes
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
Collaborates with other primary care team members to offer:
  • Case management and screening services
  • Dental screening
  • Diabetes and preventative education
  • Foot care and chiropody services
  • Insulin adjustments
  • Medical treatment
  • Nutritional counselling
  • Referrals to other health care professionals
  • Regular bloodwork
  • Retinal screening
  • Traditional healing services
Services are delivered through:
  • Individual counselling
  • Couples and family sessions
  • Group sessions
  • Community workshops
