Description of Services:

Home-centered hospice program designed to provide psychosocial, emotional and practical service to the Jewish community * services include:

counselling
care planning
resource identification
advocacy
chaplaincy
volunteer support to clients and their families
counselling for children and teens coping with parent's illness
counselling support for caregivers, caregiver relief provided through trained volunteers
bereavement support in individual and group formats

Part of a larger Jewish multi-service agency allowing clients to access numerous levels of resources/services as appropriate * cultural, religious, spiritual expertise to assess and support client * staffed by professionals including social workers, Rabbi and Volunteer Service Coordinator in partnership with Circle of Care * for clients and families who identify as Jewish * HPCO accredited program
