Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment. Once referred to Reena, Reena will arrange an initial interview, followed by a tour or the residence for acceptable applicant.
