Eligibility - Population(s) Served

Frail or cognitively impaired adults 65 years or older that needs daily access to personal care and attendant services throughout a 24 hour period everyday

Description of Services:

Personal care * assisted living services for high risk older adults to assist on activities of daily living
  • toileting
  • washing and bathing
  • preventative skin care
  • transferring, positioning and turning
  • dressing and undressing
  • assistance with eating
  • reminders re: pre-measured medications
  • exercising
  • meal preparation
  • bed making and laundry
  • light housekeeping
