Description of Services:

Operates a primary care medical clinic

Offers a walk-in and after-hours clinic

Provides access to other health care professionals within and outside of the Family Health Team, such as a dietitian, counsellors, nurse practitioners and nurses

Offers primary health care including but not limited to:

Chronic disease education and prevention (Healthy Changes)
Disease management
Health promotion
Palliative care
Rehabilitation
