Description of Services:

Diabetes management services:
  • assess your current diabetes control and medications
  • screen for diabetes complications and related risk factors
  • blood glucose meter (glucometer) training and interpretation
  • foot care services (by a Nurse or Chiropodist)
  • access to chiropody services for advanced foot care

Nutrition assessment, guidance and goal setting for:
  • improved glucose control
  • lowering blood pressure
  • improving cholesterol levels
  • avoiding hypoglycemia (low blood sugars)
  • weight management

Access to kinesiology services through partnership with local physiotherapy service provider
