Description of Services:

Assists people living on low incomes, suffering from chronic health conditions, facing mental health issues, isolation and/or language barriers

Services include:

development of individualized care plans addressing health, housing, safety needs and referrals to appropriate service providers
assistance completing applications for Canada Pension Plan, Ontario Disability Support Program, Ontario Works, passports, other identification
supportive counseling
advocating with case workers, housing and government agencies for effective solutions
