Description of Services:

Diabetes education and support in both small group and individualized settings for people living with diabetes * one-on-one or group sessions on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices
  • certified diabetes educators on site
  • off-site group diabetes education available upon request
  • workshops available from beginner to advanced learning
  • insulin starts
  • pre-existing diabetes with pregnancy, gestational diabetes
  • insulin pump therapy * certified ADP (assisted devices program) clinic
  • endocrinologists on site (doctor who specializes in the diagnosis and treatment of diseases related to hormones)
  • nutritional counselling
  • weight management
  • type 1 young adult transition program
  • advanced self-management strategies
  • health coaching for people with Type 1 and Type 2 diabetes
