Description of Services:

Medical care team focused on family and community health
  • Offers timely access to high quality primary care
  • Provides access/referrals to other health care professionals within and outside of the Family Health Clinic
  • Coordinates documentation if patient receives care from a member of the team
  • Provides a Walk-in Clinic
Offers primary health care such as:
  • Chronic disease management
  • Health promotion
  • Disease prevention
