Description of Services:
Educates patients about their medications
Utilizes the Ontario Telemedicine Network (OTN)
Works by Nurse and patient reviewing all medications with Pharmacist on the OTN who provides medication education, side effects and potential interactions
Provides a medication schedule to each patient
Records the findings in patient's Electronic Medical Records (EMR) for physician's awareness
Discharge Medication Reconciliation
Reviews all medication lists for patients discharged from Atikokan General Hospital (AGH)
Records all changes in patient's EMR
Refers patients with numerous changes to a 1-hour OTN session with Pharmacist
Also provides:
Medication reviews for patients referred to the Falls Prevention program
Monitoring of the INR test results; makes phone calls to patients with their new orders and informs them of when it's time for the next blood test
NOTE: Pharmacist makes annual visit to Atikokan Clinic for in-person consultations
