Eligibility - Population(s) Served

Residents of the City of Greater Sudbury 12 years of age or older requiring increased symptom management provoking hospitalizations, emergency room visits, physician visits, etc. * life expectancy of 12 months or less * non cancer illness or client with a cancer diagnosis no longer able to attend Northeast Cancer Centre Symptom Management Clinic (SMC) and can only be seen at home

Description of Services:

Pain and symptom consultation and emotional support to people and their family/caregivers dealing with a life-threatening illness * services can be provided at home, or in hospitals and there may be unique circumstances where our consultative services are offered in long-term care homes

CHPCT Team

  • manages the physical, emotional, and spiritual needs of clients and families, in consultation with other health care providers
  • coordinates care when required, provides education and additional support services to enable clients to live and die in the location of their choice
  • ensures seamless transition to Hospice based on client's choice
  • makes referrals to the Visiting Hospice Services to ensure volunteer support for your family
  • provides anticipatory grief services
  • provides grief and bereavement services following client's death
