Application

If deemed necessary, arrangements made upon referral from Lake of the Woods District Hospital

Eligibility - Population(s) Served

  • Individuals 65 years of age and older who have been discharged from the Lake of the Woods District Hospital and

  • are taking 4 or more medications or

  • at least 1 Institute for Safe Medication Practices (ISMP) medication

Description of Services:

After hospital discharge services include:
  • A phone call from Pharmacist upon referral from Lake of the Woods District Hospital (LWDH)
  • Assistance with identifying and resolving medication related problems that may occur during transition from hospital to home
